Hi! I would like to request information regarding:

 I am a fresh graduate student applying for Philhealth and I am also an Indigent and have a solo parent.

My question is what type of Philhealth should I apply If I come from an indigent family with solo parent?

You asked for PhilHealth Inquiry.

Response to Request

To apply as a new member of PhilHealth from being a dependent of an indigent member, you will need to follow certain steps and provide specific documents. Here's a general guide on how to do it:

Gather Required Documents:

Birth certificate (original or certified true copy)
Marriage certificate (if applicable)
Two valid IDs (e.g., passport, driver's license, SSS ID, voter's ID, etc.)
Barangay Certificate of Indigency (to prove your previous status as an indigent dependent)
PhilHealth Member Registration Form (PMRF) - You can obtain this form from a PhilHealth office or download it from the PhilHealth website.
Fill Out the PhilHealth Member Registration Form (PMRF):

Fill out the PMRF with your personal information. Make sure to provide accurate and complete details.
Visit the Nearest PhilHealth Office:

Go to the nearest PhilHealth office or PhilHealth-accredited facility in your area.
Submit Your Application:

Submit the filled-out PMRF and the required documents to the PhilHealth office or facility. Make sure to bring the original documents and photocopies.
Wait for Processing:

The PhilHealth office will process your application. This may take some time, so be patient.
Pay Your PhilHealth Contributions:

Your right to request a review

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
