Hello!
I would like to request correction of my Birthday in my vaccination record and certificate on vaxcertph. 
The website / vaxcert record for my Birthday  October 07 1995 but the correct Birthday is October 14,1995. I really need to change my Birthday asap because I have a scheduled in my agency.
