I am Arvin Austria, I have inquiries regarding my lost Philhealth ID:

1.  What are the requirements I need to present to the Philhealth office to get another ID?

2. If ever there are IDs needed, I have no other valid IDs aside from my birth certificate, is that enough?

You asked for Lost Phil Health ID.

Response to Request

To get a new PhilHealth ID, the requester may submit the accomplished PMRF with the attached (1) valid ID at the nearest PhilHealth Office.

Your right to request a review
