Hello po, good day!

I am Jenesis Mae Indino, a student. I have inquiries regarding my lost Philhealth ID:

1.  What are the requirements I need to present to the Philhealth office to get another ID?

2. If ever there are IDs needed, I have no other valid IDs aside from my birth certificate and student ID, is that enough?

You asked for Requirements for Lost Philhealth ID.

Your FOI request is approved. I enclose a copy of [all] of the information you requested. The requester may submit the accomplished PMRF with the attached (1) valid ID to the nearest PhilHealth office to get a new PhilHealth ID.
