1. Total Number of Philhealth Employer-members coming from the Formal Sector;
2. Average Headcount of Employees in the Formal Sector;
3. Geographical distribution of the Employer-members in the Formal Sector;
4. Average Philhealth contributions of Employees in the Formal Sector;
5. Compliance rate of Employer-members in the Formal Sector;
6. List of Philhealth Employer-members in the formal sector.

In particular, it would be helpful to know:
research methodology -- qualitative and quantitative, mostly for development research
use of data/information being requested --- to determine the percentage of compliance as well as the scope of Philhealth compliance in the formal sector. This will also allow me to understand the nature, size of employer-members and
their degree of compliance with Philhealth mandate.
