Hi!!

I have a covid-19 symptoms and I was about to avail RT-CPR at the Chinese General Hospital and get the philhealth discount.  However upon verification it shows that there is a discrepancy in my middle name. It was supposed to be MEÑOSA however it was encoded as MENOSA even though I have my philhealth ID issued way back 2009 they didn't honor it and ask to update my records on philhealth.

Is there any way I can update it online since I can't visit the philhealth office since I'm already on self-quarantine.

Response to Request

To update or correct your PhilHealth membership data, fill out the PhilHealth Member Registration Form (PMRF). Make sure to put a checkmark in the “For Updating” box and provide accurate information and submit the accomplished PMRF to the nearest PhilHealth office.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to <ATTY. DANTE A. GIERRAN, CPA - President and Chief Executive Officer (PCEO), nrsantiago@philhealth.gov.ph, or privacy.dpo@philhealth.gov.ph>. 

Your review request should explain why you are dissatisfied with this response and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
