I'm requesting assistance as I got married and want to update my status and last name, but some government agencies require a government-issued ID, which I don't have since I'm currently applying for one. My new last name is already listed on my PSA marriage contract, but it's causing issues as some agencies won't accept my request without a valid ID.

the following documents only I can provide with my new last name indicated.

You asked for UPDATE LAST NAME AND STATUS - SINGLE TO MARRIED.

Your FOI request is approved. I enclose a copy of [all] of the information you requested.

Please note that we are only able to provide some of the information you have requested as <request for information regarding the updating of membership status from single to married. 1. Go to our nearest PhilHealth offices/branches; 2. Submit the accomplished PhilHealth Member Registration Form (PMRF) and attach the Marriage Certificate (bring both the original and a photocopy for verification purposes). Old PhilHealth ID Card (for brides who want to change their surname after marrying in the Philippines) and bring Valid ID for backup purposes.t>.
