How can I get another PhilHealth Id because I lost my previous ID?

Greetings!

Thank you for your request dated Oct 13, 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your request

You asked for lost PhilHealth ID.

Response to your request

We regret to inform you that due to an information security incident that was detected on September 22, 2023, access to affected PhilHealth systems were disabled or unplugged as part of the information security containment measures implemented by the Corporation.  Thus, we cannot yet provide you with aa copy of your PhilHealth ID. 

We appeal for your understanding regarding this matter.  Rest assured, PhilHealth is collaborating with other partner government agencies and doing all necessary measures to address the problem and prevent its recurrence in the future.

PhilHealth will release updates on these matter via its official Facebook page.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to phic.actioncenter2023@gmailcom. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
