You asked for Need Phil Health MDF number.

Response to Request

Your FOI request is approved. I enclose a copy of [most] of the information you requested. To get a new PhilHealth ID/MDR, the requester may submit the accomplished PMRF with the attached (1) valid ID at the nearest PhilHealth Office.
