thank you for your prompt reply to my request dated August 22, 2018 regarding shockwave lithotripsy.

As per your advice as to my first foi request dated 8/22/2018, I have addressed queries 1 and 2 to DOH.  

May i additionally ask from your agency:

1. Philhealth case rate implementing guidelines for shockwave lithotripsy

2. philhealth accredited facilities offering shockwave lithotripsy in the whole country. please indicate 
A. name of facility, 
B. whether health facility is public or private, 
C. and please provide addresses of each facility

Response to your request

We were able to determine that this particular information is already available online and therefore cannot be counted as a valid FOI request. For Case Rate packages you may retrieve your requested information from https://www.philhealth.gov.ph/services/acr/. In addition, please be advised that we cannot provided lists of facilities who can provide shockwave lithotripsy alone, you may coordinate with DOH on this. However, PhilHealth accredited facilities maybe retrieved thru https://www.philhealth.gov.ph/partners/providers/

Your right to request a review
