Hi! I would like to ask for the ff:

1. Number of Z-benefit for VSD availed per region
2. Number of successful surgery under Z-benefit for VSD 
3. Number of Z-benefit for TOF availed per region
4. Number of successful surgery under Z-benefit for TOF
5. Number of patients under no-balance-billing scheme and number of patients under co-pay scheme per year.

Good day Salustiano!

Thank you for accepting my request. In your email, you stated that you will be able to get back to me with the data on or before July 13, 2017. I understand the time needed to do research, however, I would like to ask you if there is a chance I could get the data asked either end or this week or early next week, July 10 (?). The reason behind is that I will be leaving for Barcelona, Spain come July 14 and the data I asked would, hopefully, form part of the survey that I did within our community. 

My purpose in going to Spain is to join a summit that I indicated in the request. Your prompt provision of the data will really go a long way to this underserved CHD community.

P.S. I woul have had sent this request earlier than June 22 if I had not encountered errors in signing up for FOI. Turns out the jpeg format of the ID I was uploading was not being accepted by the system.
