Dear MGB: 
Good day! I would like to request a list of mining bans that were prevailing at any point between the years 2006-2018. This would include all national (e.g. EO 79 from 2012) and local (e.g. Capiz Prov. Ordinance No. 9) mining bans & moratoria that were in place during this period.

Ideally, the provided info would include the ff:
1. Brgy / Municipality / Province where mining is banned
2. Relevant ordinance, law, or EO
3. Month and year ban was passed/lifted/reinstated 
4. Duration of ban
5. Type of mining banned (all kinds; large-scale only; open-pit mining only; exploration etc.)
