Response to your request

Please be informed that your request does not fall under the FOI but under the regular frontline services of the Mines and Geosciences Bureau.

For requests concerning Geohazard Certificates, kindly send a letter of request addressed to our Regional Director through our email at region4a@mgb.gov.ph. The letter should contain the following information:

1. Purpose of the Certificate
2. Geographical coordinates of your Project site
3. Location map
4. Name and Details of Contact Person

Kindly attach pertinent documents to the request such as Transfer Certificate Title, Site Development Plan, etc. to help identify the location and extent of the project site.

We encourage you to make sure that the location of your project site is correct and accurate. MGB will not be responsible for any misinformation or wrong declaration on your part. Further, incomplete submission of documents/information will not be considered and no certificate will be issued for such cases.
