Hi! I would like to request a copy of my Form 138, which is crucial for my enrollment at Cabuyao University. I didn't finish my first year, first semester in Bachelor of Arts in Communication, and I decided to study again. I believe I didn't credit any units because I dropped my subjects. This is the last requirement I have to submit to proceed with enrollment. I don't have anyone to assist me right now, so I am requesting your kind assistance regarding this matter. Please advise on what I can do to obtain my original Form 138. Looking forward to your immediate response.
