Hi! I would like to request for request regarding:

1. Certified Copy of Affidavit of Waiver (WITH DEED OF TRANSFER OF RIGHTS) in the Register of Deeds as per entry No. 199817, Ins cp. No. 175692, page 292, Vol. 571, File 175692 dated August 31, 1995. Assignor or transferor Constantia A. Ronquillo (from Declaration cancels tax no. 11748). 

This will be use for municipal reference.
