We would like to request a copy of the whole case record, such as but not limited to the Petition, documentary exhibits , and the pleadings submitted. 

The records are needed because the Local Civil Registrar (LCR) does not want to honor the decision because the name of the Petitioner in the said case is "Cely A. Ines" but the name appearing in the Civil Registry is "Baby Cely A. Ines". We need to obtain the records of the case because we are of the modest belief that if the name appearing in the documentary exhibits is " Baby Cely Ines", it would cure the defects pointed out by the LCR.
