You: My name is Terry Hayes and I would like to request certification of notarial records for an Affidavit of Publication duly executed and notarized on July 12th 1976 by Efren O. Ramos Notary Public with PTR No. 71685. The document was notarized in Laoag City with Doc. 406, Page 83 Book VI, Series 1976. The certification shall be used for reference purposes with regard s to the authentcity of the document. I look forward to you reply.
