Hello,
I would like to inquire and request a copy of my uncle's Certificate of Live Birth. As per checking the PSA records would not be able to locate/track it, since he was born August 7, 1931. He'll be using the for the Centennial Gift of our residence local government (LGU) for he'll be turning 90 this year and for documentation purposes.
I'm hoping for your kind consideration and appropriate response to this matter.

This is to follow-up on the said request for my Uncle's Certificate of Live Birth. This will be used for his official registry/revalidation of his age for his Senior Citizen Status.
