I would like to request certification of notarial records for an Affidavit duly executed by Melvin A. Viernes and notarized on the 27th day of January 1996 by Dominador LL San Luis Notary Public with PTR no. 5806118. The document was notarized in Laoag City with Doc.No. 445, Page No. 90, Book No. LVI, Series 1996.
The certification shall be used for reference purposes with regards to the authenticity of the document.
