I am from Dr. Moises V. Cacawa Memorial Hospital.  we will be operational sometime this year.  I am assigned as the Medical Record Officer.  

As of to date, we are in the process of formulating our policies and of those policy is the Retention and Disposal.  To guide us properly, may we request for a copy of the National Archives of the Philippines DC No. 70 s. 1996 for us to have a basis in making such policy.
