Hi! I would like to request for a refund of my payment in applying NBI clearance for the following reason:

1. My last name and middle name got mixed up, instead of "Gulle" being my middle name and "Peligrino" being my last name, it was the other way around.

I'm just going to create new application so that I can change what was wrong but I already paid the fee. I hope you can help me with this one.
