Hi NBI,

Nabiktima po ako ng identity theft kung saan nagamit ang aking pangalan at phone number para mag-apply ng loan sa Online Loans Pilipinas (OLP). Pinababayaran sakin ng OLP ang loan. Gusto ko po sana malaman kung papaano po ako mag-aapply ng affidavit of identity theft at anu-ano po ang mga requirements. Salamat po.
