Until now, may mga suspicious person kaming napapansin sa negosyo namin nagbanta si queency ann tapalla, Brgy BOTE catanduanes, from ricoh company makati. Na papapatay nya buong family namin (nagbanta sya simula ng nahuli syang may karelasyon sa pamilyadong lalaki at nalaman ng lahat kaya buong pamilya namin pinagbantaan nya  dahil sa gulo ng ginawa nya. Ngayon nagaalala at nababahala  lahat dahil sa pagbabanta nya thru email . sanf pwde m at makahingi ng protection para sa mga gustong gawin nya para saamin. Kelangan po namin ng seguridad dahil sa pagbabanta nya.

Domingo, Quezon City, or to the NBI Regional or District Office nearest you and submit your complaint- affidavit together with all other documentary pieces of evidence that you may adduce on your behalf. 

You may also send your complaint via email thru crd@nbi.gov.ph. 

If unhappy with the response to your FOI request, you may ask us to carry out an internal review of the response by sending an email to foi@nbi.gov.ph Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request. 

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
