Hi, imwould like to report a alleged use of my dormant union bank account for whatsoever reasons. I lost my phone months ago, and also lost access to the said account since my number and email was on that phone. However, ive been receiving messages that It is active up to this date and being used to transfer money.

Domingo, Quezon City or to NBI Regional or District Office nearest you and bring the aforesaid documents. 
 
If dissatisfied with the response to your FOI request, you may ask us to carry out an internal review of the response by sending an email to foi@nbi.gov.ph Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.
