Hi, I would like to request for the copy of approved plan, narrative technical description OR lot description of these lots:

Lot 6, Block 1, Bsd-04-000964 (under NHA)
Lot 5, Block 1, Psd-04-000854 (under NHA)
Block 70, Psd-04-001949
ALL located at Sapang Palay, San Jose del Monte City, Bulacan. These are for survey reference purposes.
These were requested from the DENR-Surveys and MApping Division and Records Section but were not available on their file.

Greetings!
Thank you for your request dated May 28, 2021, for a copy of an approved plan and narrative technical descriptions of certain properties in Sapang Palay, San Jose Del Monte, Bulacan.
Per coordination with our Bulacan District Office, NHA only has copies of technical descriptions. However, said documents can only be released upon presentation of a notarized SPA from the awardee/s.
Attached herewith the copy of the letter reply provided to us by the said office which was also sent to your email last June 3, 2021, for your reference.
Kindly coordinate with our Bulacan District Office on this matter.
