For data privacy concerns, please submit a formal request stating your affiliation and purpose of request to the Provincial Nutrition Action Office of Batangas City at email address prohealth1@hotmail.com or pncbatangas@ymail.com.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Assistant Secretary and Executive Director IV Dr. Azucena M.
