Good day!

I would like to request a shapefile or .tiff or .csv format on the following data:
1. Infant mortality rate in the province of Leyte
2. Total Fertility rate in the province of Leyte
3. Malnutrition rate in the province of Leyte
4. Reported cases of asthma in the province of Leyte
5. Health expenditures per municipality in Leyte
6.Locations of public hospitals and botika ng bayan per municipality in Leyte.
