I am from the Data Privacy Committee of the Veterans Regional Hospital (now, Region II Trauma and Medical Center).
May I request for:
1. A PDF file of any existing Privacy Manual, preferably by another hospital, upon which we can base the Privacy Manual which we will be creating for our hospital.
Although there is a format uploaded at the agency's website, we would appreciate it if we will be provided with an already existing and NPC-approved Privacy Manual for us to be guided accurately.

Thank you for your request dated 08 October 2018 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

You requested a .pdf file of any existing Privacy Manual by a hospital which will be used as a basis for creating your own. We understand that you want to be provided with NPC-approved manual rather than the template or format uploaded on our website.
It should be stressed that the Privacy Manual is your organization's self-determined set of standards and protocols in pursuit of your responsibility to protect the privacy rights of individuals whose personal data you were entrusted with to handle. The Privacy Manual is for primary use of your organization and is not to be submitted to the NPC. Further, prior approval of NPC is not necessary, if not, inappropriate.
As such, we cannot provide you with an approved Privacy Manual from any private and public hospital as we do not have the same. The Privacy Manual Guide contained in the NPC Privacy Toolkit may still be utilized as a reference. Kindly refer to pages 82-89: https://www.privacy.gov.ph/wp-content/files/attachments/nwsltr/3rdToolkit_0618.pdf. Note that as Personal Information Controllers (PICs), you should only ensure that your Privacy Manual adequately covered your unique needs to guarantee the safety and security of personal data under your control or custody, thereby upholding an individual’s data privacy rights.

Your right to request a review
If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response, by writing to Privacy Commissioner and Chairman Raymund E.
