Hello,

A VAS permit for Internet Access Service stipulates that "Grantee shall submit to the Commission within fifteen (15) days after the end of each quarter, reports of its operation in the form prescribed by the Commission."

I would like to request the form, template and/or format of said reports, as I am unable to find it anywhere online.

I was also unable to acquire it when I visited the regional NTC office.

I would prefer that these be sent via email.

While our aim is to provide information whenever possible, in this instance this Office finds that the copy of the passport that you submitted is already expired, we can't forward your request t the Decision Maker. 

However, kindly provide us a valid copy of government issued identification in order for us to forward your request to the concerned Operating Unit or you may request directly to the Office of the Director of NTC, Regulation Branch  by writing to the following address:
