November 19, 2020 Dear Ermie Villacer, Greetings! Thank you for your request dated Nov 12, 2020 10:58:05 AM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for the blocking of your Lost cellphone. Response to your request While our aim is to provide information whenever possible, in this instance you mentioned that you visited our Regional Office in Davao City and we assumed that you filed for a request for blocking of your lost cellphone from that office. May we advise you therefore that we cannot request it twice as we ensure that our Regional Office had acted upon your request. However, we will take note of those IMEI Numbers, and we will check our database here in the Central Office of its status. You don't need to make follow up on this matter. For further assistance, you may review the FAQ section of the eFOI site: https://foi.gov.ph/help. Your right to request a review If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to: GAMALIEL A. CORDOBA Commissioner NTC Bldg., BIR Road, East Triangle, Diliman, Q.C. Your review request should explain why you are dissatisfied with this response and should be made within 15 calendar days from the date when you received this letter.
