Hi! I want to follow up on my request to block my stolen mobile phone last July 28, 2023.

I submitted my Affidavit of Loss with Undertaking last August 4, 2023, at NTC satellite office region VII.

The details indicated in the Affidavit of Loss are the following:

You follow up your request for  blocking of lost cellphone.

Response to Request

Your FOI request is approved. However, please be informed that the R.O has forwarded your request for blocking to CWPD of NTC Central Office to include in their request for blocking to all CMTS providers. Please be informed that NTC will no longer write the requestors individually due to bulk of requests daily but rest assured that all requests are catered accordingly.
