I would like to request from your office a copy of certificate of completion as evidence of my attendance during the seminar your office conducted last March 6 -8, 2019 held at Upper Story Serviced Apartments, Centro Tower Quezon City.  This is in partnership with Metro Manila Disaster Risk Reduction and Management Council through the Office of Civil Defense and signed by Regional Director, OCD NCR Romulo M. Cabantac, Jr.
