Hello. I would like to request for OMB permit so that my parcel would be released by Customs: 

  

Airway bill:  1Z4164AV0402920763 

Consignee: Ana Liza P. Wang 

Item: USB device/flash drive ( Trezor One) 

Purpose: Personal use only 

In the email with subject “Request for OMB Permit, I attached the following documents: 

- Signed letter of intent, addressed to Mr. Christian Natividad 

- Notarized affidavit of undertaking 

- Airway Bill for AWB  4161AV3PP4R 

- Invoice for AWB  4161AV3PP4R 

They wanted me to get a Registration fee P 500 , License fee P14,166 which is not intended for bu
