Hello. I would like to request for OMB permit so that my parcel would be released by Customs:

Airway bill: 4161AV3VN7K
Consignee: Arch Vincent Bullo
Item: USB device/flash drive ( Trezor One )
Purpose: Personal use only

In the email with subject “ Request for OMB Permit , I attached the following documents:

- Signed letter of intent, addressed to Mr. Christian Natividad
- Notarized affidavit of undertaking
- Scanned copy of my government-issued ID (Seaman's Book)
- Airway Bill for AWB 4161AV3VN7K
- Invoice for AWB 4161AV3VN7K
