Ako po ay dumudulog sa inyong butihing opisina para makahingi sana ng Financial Assistance para sa pambili ng gamot ng aking Tatay. Sya po ayay Stage 4 Renal Cancer at nangangailangan po sya ng gamot every month. Sana po ay matulungan nyo kami. 

Kumpleto po ang mga papeles na pwede naming i.submit kng kinakailangan. 

Sana po ay marlungan nyo kami. Maraming salamat po.
