Dear, OVP.

Good day!

I, Brito, Chelsea-Lyn would like to seek help for medical assistance for my mother, Merlita Tan who was diagnosed with stage 4 colon cancer and will undergo Chemotherapy treatment. 

We can't afford paying her chemotherapy treatment. I appreciate your medical assistance at any cost and it's very big help for my mom and my family.

We are also complete with the requirements.
