Hi! I, Rogelio Licup Buenaventura Jr., would like to request for Financial Assistance for Medical Bills of my wife, Maria Minervah Baldres - Buenaventura. She was admitted to the hospital in Sto. Niño, Bustos, Bulacan last February 11, 2022 until February 14, 2022 with the hospital bill amounting to Php 59, 932.00. Please let me know where to submit the medical abstract, bill and copy of her ID Card.

I signed and submitted a Promissory note. Please help us, we are both Persons with diaability (PWDs) and both of us are not fit to work anymore.
