Hi!  I would like to address of some mistake that has sent to your good office regarding to my sent google form to avail your program Medical Assistance  from the OVP.
NAME OF PATIENT: WILLIAM MENDIOLA DEMETRIO JR.
ADD: 104 MAGINHAWA ST. TEACHER VILLAGE EAST, QUEZON CITY

Instead of:
Name of Patient: Rolando Cac Javier Jr.
Add: 38 b Cp garcia st. Up campus. Quezon city

This scheduled appointment have been made Feb. 6, 2023. Appointment date will be on Feb. 17, 2023.

Unfortunately, we were able to determine that your request is a frontline service transaction and not a valid Freedom of Information (FOI) Request. Please be informed that the INFORMATION covered under Executive Order No. 2 pertains to official records, public records and to documents and papers pertaining to official acts, transactions, or decisions, as well as to government research data used as basis for policy development (Sec. 3). Thus, we are constrained to deny your request.
