Magandang Araw po,

Ako po si Leocel  Lambayan Maximo, nakatira sa BLK 5 LOT 20. CARISSA 3B BRGY. KAYPIAN SAN JOSE DEL MONTE BULACAN, ako po ay humihingi ng tulong pinansyal para sa aking asawa na mayroong sakit sa puso na naka admit sa ICU ng Heart Center of the Philippines noong FEBRUARY 1, 2023 hanggang sa araw na ito. Ako po ay humihingi ng tulong para magamot, at madugtungan ang buhay ng aking asawa. siya po ay si MARK VINCENT VENTURA EUGENIO, na nasa ilalim ng pangangalaga ni Dr. Emily Mae Yap.


Kalakip po nito ay ang kanyang medical abstract, at ang aming bayarin sa ospital na mula sa
