Dear Sir/Mam,

I am Richard A. Quasco of legal age, had 4 children all are studying. I'm residing at San Sebastian Kawit, Cavite. Right now I am unemployed since last Aug. 17 due to COVID-19.

Last month my wife, Layla A. Quasco has been diagnosed for having a stage 4 kidney cancer and also has a tumor at her left leg. She has been confined in a hospital at UP-PGH for the surgery. 

That’s why I’m writing you this letter to ask for your medical assistance since my family is having a hard time to support her medication and hospitalization. 

Hoping for your kind consideration.
