My name is Lovely M. De Guzman and I reside at Balibago, Lian, Batangas. I am writing you to state my inability to pay the bill amounting to P626,115.00 for the procedure Open-heart Surgery of my husband Guillermo L. De Guzman Jr. performed last September 8, 2020 at Philippine Heart Center in Manila. Though I and my husband, who earned 18 fruitful years of sevice in DepEd, are both teachers, our current income is just enough to cover basic necessities, and by paying the hospital bill at once would lead to a serious financial hardship for my family especially to our children.
