Bolanos,husband of the mentioned patient above,requesting for financial assistance amounting to 363,822.71,the amount stipulated to Promisory note I signed to Metro Health Specialist Hospital Inc.,Diversion Road,Cabid-an Sorsogon City..my wife has kidney failure with complication with other organs,she is undergoing series of dialysis..
Favorable action hereon will be highly appreciated..
