HI,

I would like to request financial assistance on my deceased father's medical bill. We still have a huge amount left to pay to UERM. This problem makes it hard for us to secure the death certificate of my father, which in turn affects our application for other financial assistance/benefits (ex. death benefit SSS), wherein the certificate is the main document needed.

Kindly assist us in this time of need.
