Hello po Good evening. Nagpasa po ako ng requirements for Medical Assistance sa Office of the Vice President Extension Office Tondo Manila. Kulang po ako ng Bank Details galing sa Hospital. Nagrequest po ako sa kanila ng Bank details pero ang sabi nila ay need daw po ng letter request galing sa OVP para magbigay sila ng bank details. Sana po may mabigyan niyo po ako ng formal letter na kailangan ibigay ng hospital ang kanilang details para makakuha ako ng Guarantee Letter para po sa aking Papa na si Falconerie Gaton Badayos. Maraming Salamat po and Godbless.
