Sir/Maam,

Im Bernardo Bautista De Vera III son of Patient Marilyn Llarenas Bautista who is suffering kidney failure/Chronic Kidney Disease Stage V Secondary to Diabetes Mellitus Nephropathy. My mother is currently undergoing dialysis procedure at La Union Medical Center(LUMED) twice a week tuesday and friday schedule. In this situation we are  financially broken and not enough to support her meds and procedures needed to maintain my mothers sistuation. We are part of indigent family in our Province Brgy. Bucayab Bauang La Union. Hopefully Sir/Maam my request will be granted Thank you Godbless
