To Whom It May Concern,

Hi, and Good day Ma'am and Sir, I request Medical Assistance for my medicines because I am still/continue to take and have to follow up check-up every week. I am also a PWD due to Moderate hearing loss and have been diagnosed with Chronic Suppurative Media, as inactive (L).  It was found that my ear drum is 70% perforated and almost gone. there are pussies inside the ear, and the form of my hearing bone is also wrong. Mahal po ang mga gamot ko at Sana po isa po ako sa inyong matulungan , Maraming Salamat po at Mabuhay po kayo!
