Hi VP Leni,
Magandang Araw po.
Kami po ay lumapalapit sa inyong tanggapan upang humingi po ng tulong financial para sa pang radiation po ng ama ng aming tahanan po. Ang tatay po namin ay nagkaroon ng cancer sa vocal cord at dahil sa pabalik balik po na balik sa hospital ay naubos na po halos yung sa philhealth (45days). Nag undergo po sya sa 2 operation po (tracheostomy at Laryngectomy) then nag chemo and radiation po. SI tatay ay isang jeepney driver at simula po nagkapandemya ay di na po na po sya nakapamasada. Sana po ay matulungan nyo po kami. Maraming salamat po.

Kailangan po ba mag submit ng medical abstract? pasensya na po, di po kasi namin alam ano po ang requirement nyo po kung hihingi po ng medical assistance po. Kumpleto naman po yung mga medical abstract po ng tatay namin na si Edgardo De Leon Ramos 66 years old po.

Sana po ay matulungan nyo po kami ng madugdungan po ang buhay ng aming ama.
