Hi Greetings.

I am Jan Marlon Domingo Gamotea a graduate of NSTP/ROTC way back in 2014-2015 in De la salle University Dasmariñas City Cavite and transfered in Panpacific University Urdaneta City Pangasinan campus year 2019.

I graduated ROTC and i have here my MS1 and MS2 certificate and my serial number is missing  im hoping for the issuance of my serial number because they will indecate it in my Transcrip of records to allow me take the Criminology Licensure Exam set on January 2024.

My Unit 267th nrotcu has already send a request for issuance and I badly need a serial number. Thank you!

Dear Jan Marlon,

Greetings!

We regret to inform you that this is not a valid FOI Request. You may proceed to or call your ROTC unit/Reserve Center where you finished your ROTC program. Present your authenticated Certificate of Completion to the Admin and Personnel Officer of the unit to retrieve your records and/or the Special/General Orders indicating your Name and AFP reservist serial number.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to paoafp@gmail.com. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
