I refer to my letter of Dec 12, 2020 08:25:40 PM about your request under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch, for Number of PTSD or Mental Health Disorder in Armed Forces of the Philippines.

As per the AFP FOI Manual, please submit a fully accomplished AFP FOI Request Form (attached) along with two valid government-issued IDs, security clearances (NBI, PNP, Barangay), and a letter from your school endorsing your research. You may find a copy of the AFP FOI Manual here:

And Where can I send the AFP FOI request form, 2 Governement IDs, clearances and school endorsing letter?

Is it PhilHealth ID and School ID (EARIST) acceptable for 2 government-issued IDs?
