To Whom It May Concern,
Good Day! 
I would like to request a serial number of my NSTP-ROTC Certificate urgently because it is requirement for my graduation. I took my ROTC last 2014-2015 at Pamantasan ng Lungsod ng Marikina. I don't have right now my Certificate of Completion because it was flooded.

Your assistance regarding this matter is highly appreciated.
